What is the GUIDE Program?

The GUIDE (Guiding an Improved Dementia Experience) program is a new Medicare benefit that provides free dementia navigation services to patients and caregivers. We are with you every step of the way, helping you manage care, connect with resources, and find the support you need.

Who Qualifies?

  • Medicare patients diagnosed with Alzheimer’s or related dementia
  • Living at home (not in a nursing facility)
  • Any stage of dementia — from early memory loss to advanced care needs

What You’ll Receive Through GUIDE

  • Personal Dementia Care Navigator
Your dedicated contact for guidance, planning, and community resources.
  • 24/7 Support Line
Help for urgent concerns anytime, day or night.
  • Caregiver Education
Training, tips, and tools to make caregiving easier, safer, and less stressful.
  • Respite Care
A Break for Caregivers, Peace of Mind for Families Caring for someone with dementia is a 24/7 job. Over time, it can be exhausting and overwhelming. Respite care gives family caregivers short-term relief while ensuring their loved one continues to receive safe, professional care.

Through the GUIDE Program, Medicare covers up to a set number of hours of respite care each year at no cost to you.

How Respite Care Works in GUIDE

Where It Can Happen:

  • In-home respite – A trained caregiver comes to your home so you can rest, run errands, or take care of personal needs.
  • Facility-based respite – Your loved one stays in an approved facility (such as assisted living or memory care) for a short period while you take a break.

Length of Care:

  • From a few hours to several days, depending on your needs and availability.
  • Can be scheduled in advance or arranged in emergencies.

What’s Included:

  • Supervision, engagement activities, and companionship
  • Assistance with meals, bathing, and medication reminders
  • Dementia-trained staff for specialized safety and support

Why It Matters:

Respite care is not “just a break.” It’s a lifeline for caregivers — helping you recharge, reduce stress, and continue caring at home without burning out. It also gives patients meaningful interaction and a safe change of environment.
